Student Absentee Forms

If your child(ren) will be away from school, please call our office or fill in one of the forms below. If the forms are not working, please call our office directly (Elementary campus: 604-522-1410, Secondary campus: 604-523-1580).

Elementary School Absentee Form

(For students in Kindergarten - Grade 6)

**An Absentee Form must be submitted for EACH DAY that a child is absent unless date range has already been indicated**
