Re-Registration Form

TO PARENTS:

Below is the re-registration form to be completed for each of your children currently enrolled at this school. Due to Government regulations, we are required to have a registration form for each student, each school year. Please fill out the form below for each child that you have attending the school. THIS FORM WILL BE USED TO ASSESS OUR STUDENT COUNT FOR THE NEXT SCHOOL YEAR, 2023-2024. It will also help us to determine how many new students we can accommodate K-12.

Kindly fill out the Re-Registration form by January 31, 2023.

a. CHILD(REN) RETURNING - If you know your child(ren) will be returning in September 2023, please fill out the form on or before January 31, 2023.

b. CHILD(REN) NOT RETURNING - If you know your child(ren) will not be returning, please give us the courtesy of filling out the form on or before January 31, 2023, noting your child(ren) will not be returning.

c. QUESTIONABLE/NOT SURE - If you are not sure your child(ren) will be returning, please fill out the form anyways. As you may be aware, we have a waiting list of families wishing to attend JKCS. It is essential we hear from you as soon as possible. New applicants are placed on a waitlist based on the number of current families who state they are uncertain of their return to JKCS.

    John Knox Christian School (JKCS) collects and uses personal information to provide your child with the best educational services. The personal information on these forms is required in order to register your child at JKCS and assist the school in making informed decisions on the appropriate placement of your child and allow the school to respond to an emergency. JKCS commits to using and storing this information responsibly and will not release this information to a third party without your verbal or written consent unless permitted under the Personal Information Privacy Act (PIPA) legislation.     PHOTOGRAPH PUBLISHING:

    Throughout the school year photographs are taken of various student activities which may be published in the school newsletter, in school annuals, or on the school website and other promotional materials.

    FIELD TRIPS/OFF-CAMPUS PERMISSION (PERTAINS TO GRADES 10-12 STUDENTS ONLY):

    Students may participate in field trips or exploratory day trips during the school year. These trips provide invaluable learning experiences for students. If activities are considered medium to high risk, an additional permission form with detailed information will be provided.

    PLEASE NOTIFY THE SCHOOL IMMEDIATELY IF ANY CHANGES OCCUR AFTER THIS FORM HAS BEEN RETURNED TO THE OFFICE. YOUR QUICK RESPONSE ENSURES YOUR CHILD’S SPOT AND ALLOWS US TO MAKE IMPORTANT BUDGETARY DECISIONS FOR THE COMING SCHOOL YEAR.
