Re-Registration Form 2024-2025

TO PARENTS:

Below is the re-registration form to be completed for each of your children currently enrolled at this school. Due to Government regulations, we are required to have a re-registration form for each student, each school year. Please fill out the form below for each child that you have attending the school.

THIS FORM WILL BE USED TO ASSESS OUR STUDENT COUNT FOR THE NEXT SCHOOL YEAR, 2024-2025. It will also help us to determine how many new students we can accommodate K-12.

Kindly fill out the Re-Registration form on or before February 4, 2024.

a. CHILD(REN) RETURNING - If you know your child(ren) will be returning in September 2024, please fill out the first form on this page on or before February 4, 2024.

b. CHILD(REN) NOT RETURNING - If you know your child(ren) will not be returning, please give us the courtesy of filling out the second form on this page on or before February 4, 2024, noting your child(ren) will not be returning.
